To receive the $75 monthly premium discount for the 2021 plan year, you and your covered spouse (if applicable) MUST have a wellness visit with your primary care physician or through a Catapult clinic visit by October 31, 2020.
**In addition, if you or your covered spouse (if applicable) are nicotine users, you MUST be enrolled in the nicotine cessation program by October 31, 2020.**
If you and your covered spouse would like to use your own primary care physician (PCP), you will need to take the attached form with you to your visit, and be sure they include the nicotine test.
Again this year, the District is providing opportunities to have a wellness visit through Catapult. All Clinics will be held at Bryant Junior High from 8:00-3:00 on the following dates::
July: 6, 9, 20, 23
August: 3, 4, 29
September: 19
October: 17
To schedule an appointment, please go to the online scheduler link below. After you enter your information, you will see a drop down menu to scroll and find the location. Ours can be found under the name - "Bryant - Bryant School District - Hill Farm Jr High (301 Hill Farm Road) District Employees Only".
***Please keep in mind that if you attend a Catapult clinic visit, you MUST enroll in the nicotine cessation program if you are a nicotine user. This is an additional step that is not automatically set-up for participants.***
